Planning for EHCPs and complex health needs requires education, health and care responsibilities to be clear. Families may coordinate with a school, local authority, NHS professionals, commissioners and care providers, but each organisation has its own duties and decision-making role. A home-care team may contribute relevant information, continuity and authorised support where commissioned.
